This free blood pressure diary helps you log home readings the way doctors want them: two checks a day, morning and evening, with two readings each time. It works your average out as you go, shows where it falls on the home blood pressure scale, and writes a summary you can copy for your doctor.
Your 7-day blood pressure check
Two checks a day, morning and evening, for 7 days. That’s what doctors want to see.
Evening check is next
Sit quietly for 5 minutes, then take 2 readings.

Keep going
Your result appears after 3 checks. One reading tells very little. A week of home readings tells your doctor much more than one office visit.
Home reading cut-offs from the ACC/AHA blood pressure guidelines (2017, Table 11; categories as in 2025 AHA/ACC). Education, not medical advice. Your doctor knows your full picture.
Summary for your doctor
A short summary you can copy into a message or show at your visit.
Keep your data safe
Your readings are stored only on this phone, never on our servers. If you change phones or clear your browser, they are gone. Make a backup copy and keep it anywhere you like.

How to take a home blood pressure reading
- Sit quietly for 5 minutes with your back supported and both feet flat on the floor.
- Rest your arm on a table at heart level and use a cuff that fits your upper arm.
- Take 2 readings 1 minute apart, before breakfast and before any blood pressure pills, then again before dinner.
- No coffee, exercise or smoking for 30 minutes before you measure.
Why home readings use different cut-offs
Readings at home are usually a little lower than in a doctor’s office. That is why the American Heart Association and American College of Cardiology use slightly lower limits for a home average: stage 2 high blood pressure starts at 135/85 at home, compared with 140/90 in the office. 130/80 means the same in both places. This diary uses the home values.
When to call for help
If a reading is above 180/120, wait at least 1 minute and measure again. If it is still that high and you have chest pain, shortness of breath, back pain, numbness, weakness, vision change or trouble speaking, call 911. Without symptoms, contact your doctor right away.
Your readings stay on your phone
Nothing you type is sent to us. The diary saves your readings only in your browser. Use the backup code to move them to a new phone. Days 1 to 3 are free; the full 7-day diary comes with the Heart Numbers Kit. Education, not medical advice.
